How to register
We have an open list and welcome requests for registration from patients living in or moving to the Practice area.
Register online
We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

The National Care Record Service (NCRS)
Important information regarding your NHS medical records – Summary Care Record
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Named GP
All patients at South Wigston Health Centre have a named, accountable doctor who is responsible for coordinating their care.
Your named doctor will be allocated to you by the practice. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
If you have a preference and would like to request a particular doctor at the practice to be your named GP please talk to one of our receptionists.
New Patient Checks
Some patients may be invited to attend a new patient check with one of our Practice Nurses or GP’s when they register with the practice. This would be dependent on your medical history.
This will allow us to gather information about your health needs as soon as possible, and gives you the opportunity to discuss any problems you may have.
It can take some weeks for your notes to arrive with us.
If you are on repeat medication, you will also need to see a Doctor or Practice pharmacist to have the medication set up on our clinical system so you can order it as usual. Please bring the right hand side of a previous repeat prescription or the packaging of the medication, to your appointment.
When you transfer between practices, it can take several weeks for your medical notes to arrive with us. It is important we start to get to know you so we can help you stay in the best of health.
Medical treatment is available from the date of registration.

Violent and Abusive Behaviour
The NHS operate a zero tolerance policy with regard to violence and abuse and the Practice has the right to remove violent patients from the list with immediate effect in order to safeguard Practice staff, patients and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we will notify the patient in writing of their removal from the list and record in the patient’s medical records the fact of the removal and the circumstances leading to it.
Organ Donation
From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
